• Confidential Parent Questionnaire
Grades 2—12 Applicants Only:
• Request for Transcripts*
• Copies of Two Most Recent School Report Cards
• Copies of any Relevant Educational Assessments
Grades 5—12 Applicants Only:
• Teacher Evaluation Form (PNAIS)*
High School Applicants Only:
• English Teacher Recommendation*
• Math Teacher Recommendation*
• Student Questionnaire
• SSS Application Submitted Online
You may request a mail-in form from
the Admissions Office if you wish.
• Completed TLA Application, including
IRS 2008 1040, all schedules and W-2's
• Response due today from all accepted applicants, including commitment form, deposit and new student fee.
Admissions decisions will be mailed March 19, 2009. Early Childhood Program class assignments will be mailed by June 1, 2009.
*These forms need to be processed by your child’s current school or teacher, then sent to SWS. As this process may take some time, we advise that you send them to the appropriate party early in your application process.
